For your convenience, we accept credit card (Visa, MasterCard,) cash, cheque or debit ( Interac) payments.
At the conclusion of your appointment you will receive a statement outlining the services that were performed.
Due to recent legislation, you are covered under the privacy act: your information through your insurance provider is confidential and therefore will not be released to our office. We urge YOU to become familiar with any dental benefits you may have. This office will not be pre-determining your basic dentistry. If you or your insurance company requires pre authorization, we will be happy to process and provide the necessary information.
"Optional" Direct Billing (Credit Card Required on file): Our office works on primary assignment. Therefore we will process your dental claim forms and have the insurance company pay us directly. This process can only be effective if you can accurately fill out the insurance information sheet provided to you.
Please understand that the patient is fully responsible for payment for all treatments agreed to by the patient. Patient is fully responsible for any fees not covered by the insurance company. Please insure that you understand your dental insurance coverage.
Payment of your treatment not covered by insurance or if you do not have insurance, is payable at time of service.
"Optional" Non Assignment (Non Direct Billing): All accounts are paid in full by you at the time of service. The insurance claim is sent off electronically by our office at the time of the appointment. Within 24-72 hrs you should receive an insurance payment by direct deposit or two weeks if you are not registered to receive direct deposit payments. Many people use this option should you have a points or rewards card.
As a courtesy to our patients, when more extensive treatments are required, we can customize a payment plan depending on cost and length of treatment. This includes Invisalign, and Orthodontic treatments.
Some procedures are deductible as health expenses on annual income tax returns. You should check with your accountant to determine whether you may benefit from such a claim.
Time reserved in the schedule is valuable to us and all our patients who require dental care. In order to maintain excellent service we require that you make a change to your reserved time at least two business days in advance. We reserve the right to charge $50.00 if notice is not given for a missed appointment.
